Dear Families,
The health and well-being of the Autism and/or Intellectually & Developmentally Disabled (I/DD) communities has been highly impacted by the COVID-19 Pandemic, and we know that families are still suffering the effects. Washington Autism Alliance is conducting a survey in order to inform the Department of Health about areas where our Autism and/or I/DD families have been impacted by the COVID-19 pandemic, and to make recovery recommendations on strategies for improving their health and well-being.
The survey should take less than 10 minutes to complete. Your participation is voluntary. If you don't want to answer a particular question, that's okay. All responses to the survey are confidential, and results will be reported to the Department of Health in an aggregate report of the survey results.
*Note: Please fill out a separate survey for each member of your family that experiences Autism and/or I/DD.
